+ What do I need to bring on my first visit?

- Care-card
- Up to date medication list (or the medications themselves)
- New patient registration form
- If you know why you’re coming, please fill out a condition specific questionnaire to make your visit more productive.
+ How do I obtain a refill of my prescription?
- See your family doctor for refills
- If urgent refills are required, short term can be provided for the medications we have prescribed for you. We may ask to see you again to re-evaluate your condition and determine if you should remain on the medication.
+ How do I have the physicians complete forms for time off work or insurance purposes?
We can only fill out forms for conditions directly related to what we are treating you for. You may drop them off at our Richmond Office or Fax at any time. Please complete the patient portion. There may be a nominal fee for completion and copies of relevant medical records. Visit our contact page for contact details.
+ Do I need a referral from my primary care/family physician?
Government regulations require that you have a referral from you family doctor to see us, unless you have been seen in our office within the last 6 months. Please contact your family doctor directly.
+ Do you have forms online to so that I can fill them out before coming into the office?
